Personally, I always put a small piece of electrical tape on the inside of the peephole....and I put up the Do Not Disturb sign pretty much for the whole stay, and maintain the room myself. We stay at nicer places, and also ask for extra pillows and towels up front. We're in the loyalty programs, and stay in the same couple of chains (typically Hilton, Marriot, and Loews hotels). We also leave a gratuity in the room when we check out (of about $5 per day of our stay...mostly because they are only cleaning when we leave, not during the stay). We always Lysol places where we'll eat or do hygiene activities, just as an extra step.
Most of the time we're at a hotel, it's due to a convention, and we've got really expensive props, costumes, etc., that we'd rather not have folks in the room at all.
Any meds, or small valuables go in the safe.
