Why does my new to me doctor require this information: SAMPLE QUESTIONS.

In the last 12 months.........

Has anyone hurt your feelings??

Has anyone talked "DOWN" to you??

Has anyone Yelled at you??

Do you have any education??
 
Where does this information go..??? It some place mentioned "CDC". But I'll bet it is accessible to many way-way far outside the medical community.

While at the new to me doctor, he mentioned past PSA test "results" over several years. I asked where he got that information, as I had only given, results from a PSA test a month ago. He just pointed at the computer and said, "It is here". I did not push my question further.

Why does my Urologist need that information.....??? Let's say hypothetically someone has "lost it". Does my Urologist make that determination, who does he notify to fix "social" problems.

I am quite surprised there is not a question about firearms in your house/home/dwelling. Is there a Social Determinants Judge and jury. Is there a police force charged with making an in-home assessment, what powers do they have....???
 
Depends on your state, I suppose. They mostly save the firearms question for the kids. They speak to the young teens without a parent in the room and ask if there's guns in the house.
I convinced my mom to go to assisted living from living in her own apartment because of what I was seeing....memory issues, constant falling (where I'd have to take her in for stitches), not eating properly and losing weight too fast. She did a whole lot better in assisted living, and still had her own apartment, but had someone checking on her constantly and making her meals and doing the laundry and cleaning.
So I'm going to guess that a doc is asking questions along those lines to determine memory, falls, keeping up with health, and if it appears you aren't taking care of yourself, then they call Senior Services.
 
Husband had some new questions asked to him recently, and he asked why, and the nurse said it's for Medicare and they need to ask each year. About 30 questions. I remember one was asking if you knew your health condition. Another one asked you to rate your health against someone your same age...is it better, worse
And of course the regular are you depressed, want to hurt yourself, do you fall
 
Since I turned 65 my doctor gives me some stupid questioner to fill out. My standard response is to answer NO on everything. Of course they like to put in trick questions where it takes a YES answer to say no. Some of the questions are math questions or story problems. The doc explained that it's to check for old timers mental problems.
 
I suspect not.

Those questions are focused at finding abusive situations.

And yet...........Getting a new card (even applying for a new card) or canceling a credit card effects your credit score. As does if you move to a different location. I think your driving record also effects your credit score. Likely much other information. A list of other things:

If they are a connected medical group they see it all....primary care to specialists

when i had my bad wreak 30some years ago they did ask me about my religious preference......instantly even though i was hurt i asked why and what are my options...i didnt want to answer but they forced me sorta...they asked why so hesitant and i said because that will be a record forever and what if 30 years from now the nazis are in charge and round up christians for the chambers here...their jaws dropped open !!
 
Just give them the answers they want to hear. Unless you want to go into the "home." ;)

"Do you have any education?" I'd feel obligated to answer, "No, I can't read either."

The one I hate is, "list prior surgeries and illnesses... and they give you one line to list them on."

Seriously? I'm 70 years old, it's a miricle I'm still here. My surgeries and medical records fill a file box.

As for guns, the answer is: "Guns are bad, I don't believe in guns, I would never allow one in my house." I'm doubly screwed, my surgeon belongs to my gun club... and I bring him a box of ammo every so often, like how other patients bring cookies. :D

A urinary tract infection isn't a social problem but if a UTI is serious enough and/or goes undetected long enough it can cause memory loss, delirium and dementia, especially in older people. If dementia is suspected a urologist can make the determination about whether or not the dementia and associated mental problems is reversible due to proper treatment of the UTI.

If the patient does not have a UTI then that can be ruled out as a cause of the suspected dementia. Then the patient gets referred to some other medical practitioner whose field of expertise lies in mental health issues and their causes.

I was confirming that yes, your urologist CAN make that determination and refer you to somebody else.

Don't be concerned Sourdough, I didn't take offense to your previous comment and I don't think you meant to be disrespectful. I know what you are like and I can make like a duck with water running off its back when you make knee-jerk responses. I like you and think you're a hoot and I don't think you're any more demented today than I sometimes thought you were 17 years ago when you always found my posts confusing to you back then too. ;):)
